ഗുരുഗ്രാം: നെഞ്ചിൽ 1.9 കിലോഗ്രാം ഭാരമുള്ള അപൂർവ്വ ട്യൂമർ ബാധിച്ച പതിനേഴുകാരനെ ജീവിതത്തിലേക്ക് തിരികെ കൊണ്ട് വന്നിരിക്കുകയാണ് ഗുരുഗ്രാമിലെ ഫോർട്ടിസ് മെമ്മോറിയൽ റിസർച്ച് ഇൻസ്റ്റിട്യൂട്ടിലെ ഡോക്ടർമാർ. ട്യൂമർ വിജയകരമായി നീക്കം ചെയ്‌തതായി ഇവിടുത്തെ ഡോക്ടർമാർ അറിയിച്ചു. കടുത്ത നെഞ്ച് വേദനയും പനിയുമായി രോഗി ഫോർട്ടിസ് ആശുപത്രിയിലേക്ക് എത്തുകയായിരുന്നു. വിവിധ ടെസ്റ്റുകൾക്ക് ശേഷം രോഗിക്ക് തൈമോലിപോമ എന്ന ഒരു അപൂർവ്വ തരം ട്യൂമർ ആണ് ബാധിച്ചിരിക്കുന്നത് എന്ന് ഡോക്ടർമാർ മനസ്സിലാക്കി. തൈമസ് ഗ്രന്ഥി വലിയ രീതിയിൽ വളരുകയും ഇത് നെഞ്ചിൻ്റെയും ശ്വാസകോശത്തിൻ്റെയും വിപുലമായ ഭാഗങ്ങളെ മൂടുകയും ചെയ്യുന്ന അപകടകരമായ ട്യൂമർ ആണ് തൈമോലിപോമ. ഇത്തരം കേസുകൾ കൃത്യസമയത്ത് ചികിത്സിച്ചില്ലെങ്കിൽ, രോഗിക്ക് ശ്വാസതടസ്സം, അട്രോഫിക് ഡയഫ്രം പേശികൾ (ഡയഫ്രം കൃത്യമായി പ്രവർത്തിക്കാതിരിക്കൽ) , ശ്വാസകോശത്തിൻ്റെ ശേഷി കുറയൽ തുടങ്ങിയ ഗുരുതരമായ സങ്കീർണതകൾ ഉണ്ടാകാമെന്ന് ഡോക്ടർമാർ പറയുന്നു. സി.ടി സ്‌കാനിൽ രോഗിയുടെ നെഞ്ചിൻ്റെ അറയിൽ 1.9 കിലോഗ്രാം ഭാരമുള്ള ഒരു വലിയ ട്യൂമർ ഡോക്ടർമാർ കണ്ടെത്തുകയായിരുന്നു. നെഞ്ചിൻ്റെ അകത്തെ ഒരു വലിയ ശതമാനം ഭാഗവും ഈ ട്യൂമർ മൂടുകയും ചെയ്‌തിരുന്നു. ഈ ട്യൂമർ രോഗിയുടെ ശ്വാസകോശത്തിലും ഹൃദയത്തിലും വലിയ പ്രഷർ തന്നെ ചമത്തുന്നുണ്ടായിരുന്നു. ഇതിൻ്റെ ഫലമായി ഹൃദയത്തിനും ശ്വാസകോശത്തിനും കൃത്യമായി പ്രവർത്തിക്കാൻ കഴിയുന്നുണ്ടായിരുന്നില്ല. അതിനാൽ ഉടൻ തന്നെ സർജറി ചെയ്‌ത്‌ ട്യൂമർ നീക്കം ചെയ്യാൻ ഡോക്ടർമാർ തീരുമാനിക്കുകയായിരുന്നു. ശ്വാസകോശത്തിൻ്റെ വശങ്ങളിൽ മുറിവുണ്ടാക്കിക്കൊണ്ട് ട്യൂമർ ആക്‌സസ് ചെയ്യുന്നതിനും നീക്കം ചെയ്യുന്നതിനുമുള്ള അപകടസാധ്യതകൾ ഡോക്ടർമാരുടെ സംഘം തന്ത്രപരമായി കുറച്ചുവെന്ന് ആശുപത്രിയുടെ പ്രസ്താവനയിൽ പറയുന്നു. ഈ സർജറിയുടെ ഏറ്റവും പ്രധാനപ്പെട്ട കാര്യങ്ങളിലൊന്ന് രോഗിക്ക് അനസ്തേഷ്യ നൽകുന്നതാണെന്നു ഡോക്ടർമാർ പറഞ്ഞു. അനസ്തേഷ്യ നല്‌കുമ്പോൾ ഹൃദയം ഞെരുങ്ങൽ രക്തചംക്രമണം നിലയ്‌ക്കൽ തുടങ്ങിയ അപകട സാധ്യതകൾ ഉള്ളതിനാൽ മെഡിക്കൽ സംഘം ഈ പ്രക്രിയ വളരെ കൃത്യതയോടെ തന്നെ ചെയ്തു. അങ്ങനെ ഒരുപാട് അപകട സാധ്യതകൾ ഉണ്ടായിരുന്ന ഈ സർജറി വിജയകരമായി തന്നെ ഡോക്ടർമാർ ചെയ്തു. "തൈമോലിപോമകൾ ഫാറ്റി ടിഷ്യൂകളും തൈമിക് ടിഷ്യുകളും ചേർന്ന അപൂർവ ട്യൂമറുകളാണ്. തൈമോലിപോമകൾ ഗണ്യമായ വലുപ്പത്തിലേക്ക് വളരുകയും ചുറ്റുമുള്ള ഘടനകളുടെ കംപ്രഷൻ ഉണ്ടാക്കുകയും ചെയ്യും. ഇത് ശ്വാസോച്ഛ്വാസം, നെഞ്ചുവേദന, മറ്റ് അസ്വസ്ഥതകൾ എന്നിവയിലേക്ക് നയിക്കുന്നു. ഫോർട്ടിസ് മെമ്മോറിയൽ റിസർച്ച് ഇൻസ്റ്റിറ്റ്യൂട്ടിലെ കാർഡിയോതൊറാസിക് ആൻഡ് വാസ്കുലർ സർജറി (സി.ടി.വി.എസ്) ഡയറക്ടറും തലവനുമായ ഡോ.ഉദ്ഗത് ധീർ പറഞ്ഞു. ''ട്യൂമറിൻ്റെ വലിപ്പം രോഗിയുടെ ജീവിത നിലവാരത്തെ കാര്യമായിട്ട് തന്നെ ബാധിച്ചു. വിപുലമായ ഇമേജിംഗ് ടെക്നിക്കുകളും കൃത്യമായ ശസ്ത്രക്രിയാ ആസൂത്രണവും ഉപയോഗിച്ച് തൈമോലിപോമ നീക്കം ചെയ്യാൻ ഞങ്ങൾക്ക് കഴിഞ്ഞു. ഒപ്പം വീണ്ടും വളരാൻ സാധ്യതയുള്ള എല്ലാ ടിഷ്യൂകളും ഞങ്ങൾ നീക്കം ചെയ്തു. അതിനാൽ വീണ്ടും രോഗിക്ക് ട്യൂമർ വരാൻ സാധ്യതയില്ല." അദ്ദേഹം കൂട്ടിച്ചേർത്തു. ഈ വാർത്തയറിഞ്ഞ ഫോർട്ടിസ് മെമ്മോറിയൽ റിസർച്ച് ഇൻസ്റ്റിട്യൂട്ടിൻ്റെ സീനിയർ വൈസ് പ്രസിഡണ്ടും ബിസിനസ്സ് തലവനുമായ മഹിപാൽ സിംഗ് ബാനോട്ട് അതീവ സന്തുഷ്ടനായിരുന്നു. Among the foremost hurdles encountered in TKR for overweight patients are the elevated risks associated with anesthesia and the heightened potential for embolism. This underscores the importance of precision and vigilance in addressing the specific needs of each patient to ensure optimal results and minimize complications.
Furthermore, the conventional use of a tourniquet, a standard protocol in TKR surgeries, presents technical challenges due to the patient's larger thigh circumference, heightening the risk of tissue damage and post-operative complications. In response to these challenges, Dr. Nawalkar collaborated closely with instrument manufacturers to devise specialized instruments tailored to accommodate the patient's unique anatomy, thereby ensuring optimal surgical outcomes. Additionally, meticulous preoperative assessments and consultations were conducted to ascertain the patient's readiness and obtain informed consent.
Richard Koszarek, the patient, underwent the surgery on January 7, 2024, with no reported complications during the procedure. However, the path to recovery was equally crucial, with rehabilitation playing a pivotal role in achieving successful outcomes. Despite facing reluctance from numerous doctors in India and abroad due to his weight, Richard expressed gratitude to the surgical team for their unwavering commitment and skillful intervention, which enabled him to regain mobility and resume an active lifestyle.
